I have a brutally honest approach, and often tell prospective clients that they don't have a remote chance of ranking, either due to the highly competitive nature of their niche, or because of their unrealistic suggested budget.
Many many "others" are completely happy to accept new business even when they know that any gains and improvement are going to be negligible. I do not, and end up turning away 80-90% of all new inquiries. Instead, I prefer to under-promise and over-deliver, rather than take clients that I know cannot be helped.
