Nothing. This was possibly one of the worst designed user interfaces I have ever seen.

Everything. The user experience is horrific, the privacy policy is basically written so they can do anything with your data, and your social is tied to this forever. There is absolutely no recourse. Insurance companies can use this information to discriminate. HIPAA does not apply to your employer.

This is an absolute abomination. The user interface is garbage and was designed by someone with no knowledge of how humans think. It is BEYOND frustrating to fill out. On one of these reviews, someone wrote they "would rather handwrite a dictionary," and that is the most accurate description I can think of to describe this. This form is INVASIVE. This asks for your medical history from the time you were born until now and if you do not submit everything (with specific dates, diagnoses, and dosages, including things like birth control or really anything at all), they can elect not to cover you. If you refuse to fill this out, you cannot get health insurance through your employer. Absolute disaster, this should be illegal.
