Integrating Human Factors into the Design and Operations Risk Framework

Failure to answer and provide the required controls to address questions, have led to catastrophic major accident events, resulting in major damage to the company's reputation, combined with financial consequences that threaten the capacity of the company to recover financially. The investigation of these incidents, such as NP's Deepwater Horizon well blow-out accident, often identify failings in the human and organizational capabilities to manage  the major hazards that exist during the facility development and/or operation.
